From Cortez:
Go west on Main Street. When Main Street ends at Highway 491, follow Highway 491 south towards Towaoc, Four Corners, Shiprock. Where 491 merges from two lanes into one lane just past the Johnson Building, turn right onto McElmo Street. Immediately turn left onto the frontage road and continue south until you see the Hospice of Montezuma billboard. Turn right onto the driveway just south of the billboard. Hospice is at the top of the driveway.
From Towaoc:
Follow Highway 491 north towards Cortez. Continue past the Port of Entry. On the west side of the highway there is a auto sales business “4×4 Auto Sales”. Just north of 4×4 Auto Sales there is an exit off of the highway onto a frontage road. Exit onto the frontage road on the west side of the highway. Immediately turn to the right (north) and continue up the hill. Turn left (west) onto the driveway off of the frontage road. Hospice is at the top of the hill.
